       1. Passport & photocopy*4 護照及護照影本*4

       2. Republic of China (Taiwan) Visa & photocopy*4 簽證及簽證影本*4

       3. Authenticated proof of the overseas medical and injury insurance valid for at least six months from

           the date entering Taiwan 驗證後自入臺日起至少6個月以上效期的海外醫療保險證明

       4. Authenticated health certificate for short-term students 驗證後短期研修生健康檢查報告書

       5. 2-inch headshot photo (3.5cmx4.5cm)*4  白底2吋大頭照(3.5cmx4.5cm)*4

       6. Admission letterr 入學許可函

◆  Students holding the resident visa 取得居留簽證者

       1. Passport & photocopy*4 護照及護照影本*4

       2. Republic of China (Taiwan) Visa & photocopy*4 簽證及簽證影本*4

       3. Authenticated proof of the overseas medical and injury insurance valid for at least six months from

            the date entering Taiwan 驗證後自入臺日起至少6個月以上效期的海外醫療保險證明

       4. The health certificate for residence application (please keep one photocopy before making

             your visa application) 居留簽證健康檢查表影本 (申請簽證時的必備文件，請先留一份影本)

       5. 2-inch headshot photo (3.5cmx4.5cm)*4  白底2吋大頭照(3.5cmx4.5cm)*4
